Why Do We Make You Fill Out These Forms?
We want to know your pet's behavioral therapy is working and whether it needs to be adjusted before refilling.
Note: We can only prescribe medications included in your animal's behavior treatment plan. All other medications will need to be prescribed by your primary veterinarian.
Every 6 months- In-Person Recheck Required
If your pet has not been seen by us in the last 6 months, we cannot refill the medication.
Please call your veterinarian for a refill and schedule a recheck appointment with your behavior vet if your pet needs continuing behavioral care.
- If your pet is on a controlled substance such as phenobarbital, alprazolam/Xanax, diazepam/Valium, clonazepam/Klonopin, etc., due to governmental regulations you will need a paper prescription slip to give to your local human pharmacy. Refills are not allowed on these scripts, and the scripts can only be written for 30 days of medication. The scripts expire in 30 days from the date of the script. Controlled substance prescriptions cannot be called into a pharmacy except in emergencies and then only for 5 days followed up with a paper script for the 5 days.
- Because controlled substance prescriptions require such special handling, you should request a refill at least 7 business days before you will run out of this medication. This is especially important if your pet is on this type of medication every day because he/she may not be able to be taken off of the medication abruptly without the potential for side effects.
