Initial Assessment
We highly recommend you complete and return a history form (see below) to us 24 hours before an Initial Assessment appointment. This way we'll have more time to talk about all of the issues and discuss treatment. You, the owner, should come in for the appointment with your pet. Please do not send a friend or staff member to be the sole family member accompanying your pet. Of course, they may come with you if they are involved in your pet's management.

Recheck Appointment
We recommend you complete and return a recheck history form (see below) to us 24 hours before any recheck appointment. This way we'll have more time to talk about progress with the current treatment plan and update the plan to increase success. Send these documents at least 3 days before the appointment:
- Medical records from your primary care veterinarian
- Most recent lab work including CBC, UA, chemistry panel and thyroid
- A completed history form from above (if not completed already)
- Any written training plans or notes from previous trainers or behaviorists
Also, please bring as many of the following as possible:
- Any written training plans or notes from previous trainers or behaviorists
- Your pet's favorite treats, toys, or games
- Your pet's previous training tools (leashes, collars, muzzles, etc.)
- Your pet (on leash or in a carrier)
- Dogs that have bitten people or other animals should wear a muzzle until they get into the clinic room and Dr. C. gives the OK
- Video of the behavior problem
(NOTE: DO THIS ONLY IF YOU CAN GET THE FOOTAGE WITHOUT CREATING A RISKY SITUATION FOR OTHER PEOPLE OR ANIMALS.)
